Description

Looking to build consistency in your SEND referral process and give staff a clear point of reference for how referrals fit within your school’s overall SEND provision?

This Cause for Concern Flowchart will support you and your colleagues to understand and follow the same robust approach to identifying SEND need and securing support. Each stage aligns with the graduated response (assess, plan, do, review), supporting a consistent approach and helping to ensure that appropriate provision is put in place for pupils who need additional support.

Use it in SEND meetings, include it in induction materials, or display it in the staffroom to ensure confidence and consistency in approach.
